Insurance Information 

We participate in most major medical insurance plans including Cigna, Coresource, BCBS, Aetna, Humana, PHCS, UHC and Medicare.

It is always recommended to contact your insurance plan to confirm our physicians are in network with your plan prior to your visit.

We are a Medicare provider but have limited Medicare Advantage Plan participation. Please call your carrier to confirm we are in network with your plan.

We are NOT a Medicaid provider. This includes BCBS TNCare or UHC Community Plan.

What to Expect

Please bring insurance information and identification to your appointment. Health information is collected through Phreesia and a link to the forms will be emailed, texted, or given in the office to complete prior to your appointment.
